Practice Policy
Welcome to Gunter Family Medicine — where your health and comfort are our top priorities. Please take a moment to review our practice policies to ensure a smooth and respectful experience for all patients.
Appointments
Appointments are strongly encouraged, as they help streamline our workflow and ensure you are seen as efficiently and timely as possible. Walk-ins are welcome and will be accommodated based on availability. We strive to run on time and ask that you arrive 15-30 minutes early for your scheduled appointment, depending on whether you have already filled out your online patient intake forms. If you are more than 15 minutes late, we may need to reschedule.
To cancel or reschedule, please notify us at least 24 hours in advance.
- A non-sufficient funds (NSF) fee of $35.00 per returned item
- Any fees charged by the financial institution
- A late payment fee of up to 1.5% per month (or the maximum allowed by
- state law) on unpaid balances
- iv. A missed appointment fee of $50.00 for the first, $100.00 for the second,
- $150.00 for the third, $250.00 for all missed appointments beyond (or the
- maximum allowed by state law).
New Patients
New patients are welcome! Please arrive 30 minutes early for your first visit to complete necessary paperwork, or complete your new patient intake forms online to make it even easier via our website. Please bring a valid photo ID, insurance card, and a list of any medications you are currently taking.
Insurance and Payments
We accept most major insurance plans. Please verify coverage with your provider before your appointment. Co-pays, deductibles, and any charges not covered by your insurance are due at the time of service.
For uninsured patients or those with high-deductible plans, we offer competitive self-pay rates.
Prescription Refills
Prescription refills should be requested through your pharmacy. Please allow 48 hours for processing. Routine refills are only handled during normal business hours. Refills may require an office visit depending on your condition and medication type.
After-Hours Care
If you have an urgent medical issue after hours, call our office number for guidance. In the event of an emergency, please call 911 or go to the nearest emergency room.
Medical Records
To request your medical records, please complete a written authorization form. Allow up to 7–10 business days for processing. Records may be released to you, another provider, or a facility per your request.
Respectful Environment
We are committed to providing a welcoming and respectful environment for all patients, staff, and visitors. Abusive, threatening, or disruptive behavior will not be tolerated and may result in dismissal from our practice.
