Important Information for Patients with HMO Insurance Plans

If you are enrolled in a Health Maintenance Organization (HMO) plan, your insurance company requires you to designate a Primary Care Provider (PCP) before your visit. To help avoid claim denials or unexpected out-of-pocket costs, please make sure that the provider you plan to see at Gunter Family Medicine is selected as your PCP prior to your appointment.

Because insurance companies require this designation to come directly from the member, our clinic is not permitted to make this change on your behalf. Updating your PCP is typically quick and can be completed through your insurance plan’s member portal or by calling the customer service number on the back of your insurance card.

We are here to support you, and if you have any questions about this process, our team is happy to help guide you on what to ask your insurance plan. Our goal is to ensure your visit goes smoothly and your benefits are processed correctly.
